OHC’s Resource Center consists of social patient advocates working on behalf of their patients to make a significant clinical impact by addressing their social needs. There are several external factors that affects a patient’s health and wellbeing, including income, education, physical environment, culture, etc. For example, higher income is linked to better health. The greater the gap is between the richest and poorest people, the greater the differences in health. Our Resource Center is here to bridge the gap between social disparities and health. We provide assistance in transportation, food, housing, insurance, fitness programs, prescription savings programs, etc. Patients are referred to the Resource Center from either our MTM program or Clinical Partners.
