What is MTM?

MTM optimizes therapeutic outcomes through evaluating the safety, appropriateness and efficacy of medication treatment plans. It involves personalized patient consultations designed to engage patients in their own health, improve patient care, resolve medication-related problems, enhance communication between patients and providers and improve collaboration among healthcare teams.

Why is it Needed?

Medication-related problems and medication mismanagement are a major public health problem in the United States. Experts estimate that 1.5 million preventable adverse events occur each year that result in $177 billion in medication-related morbidity and mortality. MTM services have been very successful in helping patients and physicians meet clinical goals, saving money for patients and health plans and improving overall quality of life. In recognition of the value that MTM provides in improving overall health, many health plans offer this service to Medicare and non-Medicare patients. Pharmacists are in the perfect position to provide these valuable services that can complement primary care.
