Optimal Health Foundation (OHF) is a non-profit community-based organization comprised of highly dedicated health care professionals devoted to improving the health and quality of care of people. It is composed of several programs that address and resolve major issues plaguing urban communities, which include poor health, low quality healthcare service, lack of resources, poverty and low education.

OHF was founded by two pharmacists in Miami, Fl. As community pharmacists they constantly witnessed different challenges that affected the health of their family, friends, patients and people in the community. By working in community pharmacy, they were able to have a positive influence on many of their patients’ health, especially when they are able to go above and beyond their traditional job duties. Limited by corporations' policies, additional job responsibilities and time constraints, they wanted to create an opportunity where every patient-interaction is dedicated to personalized attention.

The pharmacists at facilitate valuable interactions between patients, physicians, health care professionals, medical facilities and health plans to provide patient-centered care. Through our one-on-one MTM consultations with patients, we are able to manage medication-related problems, consult physicians, provide disease management education, connect patients to additional social resources and much more.

Our innovative programs are designed to engage more patients and increase healthier outcomes. We provide disease prevention and management workshops to enable people to be pro-active and self manage their health. OHF believes that it is very important to manage other areas of a person's life in order for a person to achieve their optimal health.
