Optimal Health Consultants (OHC) truly believes that having a comprehensive and connected healthcare team may have a positive impact on the patients’ health. OHC partners with healthcare providers and/or systems in order to deliver exceptional patient-centered care. We offer many services to our partnered clinics including Medication Therapy Management (MTM) services to their patients, continue education seminars to staff, patient chart reviews, assistance with formulary development, etc. OHC’s MTM program is specifically designed to integrate our pharmacists with the patients’ entire healthcare team including their PCP, physician assistant, specialists, nurses, nutritionist, social worker, etc.

MTMs are personalized consultations aimed to optimize therapeutic outcomes through evaluating the safety, appropriateness and efficacy of medication treatment plans. There are several positive attributes that MTM services can offer by being integrated as part of the total patients’ care. Some of the benefits that MTM offer are improving adherence, preventing dangerous drug interactions, reducing hospital admission rates, etc. Refer to the MTM section for more details about OHC’s MTM program.

The patients at our partnered-clinics have the choice of receiving face-to-face MTM consultations onsite at their medical facility or in the comfort of their own home over the telephone. Both options may have a similar impact on improving the patient’s therapeutic outcomes. However, one option may be more beneficial for certain patients depending on their preference or circumstances.
Since OHC’s MTM program is based on a collaborative design, our pharmacists are able to easily collect the results of laboratory tests and values of clinical parameters that may be crucial to a positive health outcome. Vice versa, our pharmacists may easily consult with the providers regarding any therapy recommendations or significant information.
In addition, OHC understands that some patients may have external social factors that are negatively impacting a patients’ health. Therefore we can refer eligible patients to our Resource Center for additional help with social barriers, such as transportation, food assistance, etc. Please refer to our Resource Center section for more details.
